Before you fill out the form, please note the following:

1. Unfortunately, we do not accept Medicaid at this time. We appreciate your understanding and encourage you to explore other options that may be available to you.
2. The only insurance we are curently in network with is Medicare (we used to be in network with Blue Cross Blue Shield as well, but are not anymore).
3. Some insurance plans are eligible for out of network benefits that may provide reimbursement for our services. We are happy to look into potential out of network benefits and - free of charge - submit to insurance carriers on our patient's behalf. However, we cannot guarantee what reimbursement is actually issued.
